Services
Home and community support services (HCSS)
For individuals who require assistance with personal care tasks, and whose health and welfare in the community is at risk because supervision is required when no personal care task is being performed. Services are provided under the direction and oversight of a Registered Professional Nurse.
Licensed health care services for individuals with Traumatic Brain Injury (TBI)
For individuals who have acquired injury to the brain caused by an external physical force, resulting in total or partial functional disability or psychological impairment, or both, that adversely affects an individual's performance in one or more areas, such as cognition, language, memory, attention, reasoning, abstract thinking, judgment, problem-solving, sensory, perceptual and motor abilities, psychological behavior, physical functions, information processing, and speech.
Licensed health care services for individuals enrolled in the Nursing Home Transition and Diversion Waiver (NHTD) Program
Home visits by medical personnel, independent living skills training, wellness counseling, community integration counseling, nutritional counseling and education. Personal care services for individuals who require assistance with activities of daily living and other tasks and whose health and welfare in the community is at risk because supervision of the participant is required when no personal care task is being performed. Services are provided under the direction and supervision of a Registered Professional Nurse.
Nursing services
Wound care, nutrition and diabetic counseling, monitoring of blood pressure and other vital signs, medication management and reminders.
Home health aide services
Assistance with bathing and dressing, medication reminders, home exercise programs, accompaniment to medical appointments.
Homemaker and housekeeper services
Meal preparation, assistance with errands and shopping, light housekeeping, laundry and chores.

Our Director of Community Services is a Registered Nurse Practitioner with extensive experience meeting the social, emotional, environmental and medical needs of individuals with disabilities and seniors.
Home care services are available 7 days a week, including weekends and holidays, for up to 24 hours per day.
